The Manage Application Forms page enables jurisdictions and Avolve Professional Services to upload CSS, JavaScript, custom forms specific to the jurisdiction. Each form is delivered as a package. The upload must be a zip folder. This section is for custom forms and not FormsBuilder.
Form information is editable
by selecting the Edit icon. You can
add a new application form by naming the form, and uploading the file.
Forms can be created in any IDE (Integrated Development Environment) of
your choosing. An
IDE is a software application used by programmer for code development
and normally consists of a source code editor, a compiler/interpreter,
and a debugger.
The forms must be coded to the .NET framework, at this time the current version is 4.5.2
It is not recommended to upload during regularly scheduled maintenance hours, as uploading files onto the server can cause the site to reset and cause potential errors for active users.
Active
Scripting must be enabled for the validation on the eforms to work
in Internet Explorer.
The Type of form radio selection, either Custom Template or FormsBuilder, is critical to the successful operation of the form. If a custom form is designated as a FormsBuilder form in error, the custom form will error out.

The Format radio, Intake, is most commonly used.
At this time Integration Form radio is only used for uploading inspection forms that are launched in PlansAnywhere.
